We help patients navigate life after a stroke. Stroke recovery involves changes in daily habits, medicines, rehabilitation, and sometimes surgery to lower the risk of another stroke.

Medication Management

Stroke patients are often prescribed blood-thinners as well as blood pressure and cholesterol medications. They may also need assistance remembering and understanding dosing instructions.

Rehabilitation

Stroke rehab may include speech, occupational, and physical therapy. Our therapists help patients regain strength, mobility, and communication and relearn activities of daily living.

Tools for Recovery

We help stroke survivors continue to live independently through home safety assessments, care coordination, and educating patients and loved ones about how to prevent stroke recurrence.
